All staff, employees, and contractors of Tennis Master LLC are required to maintain the confidentiality of any client, student, or internal business information accessed during their duties. This obligation continues even after the end of their relationship with the company.

terms & conditions
1. TENNIS MASTER LLC will invoice the Client on the first (1st) of every month unless otherwise
noted in the Payment schedule. The Client agrees to pay the invoice(s) by the due date(s)
specified. Unpaid or overdue invoices after three (3) days may result in suspension, termination,
and the charge of $100 of outstanding fees.
2. There is a registration fee of $50
3. If service is completely terminated and the Client decides to register again, the same
registration fee of $50 will be charged again.
4. Our only method of payment is by using credit cards. There is a service fee of 3%.
This recurring charge is to cover the cost of tennis lessons.
I authorize TENNIS MASTER LLC to charge and keep my credit card on file on [1ST OF EVERY
MONTH] and for the same amount on a monthly basis thereafter.
I authorize TENNIS MASTER LLC to keep my credit card on file for any future transactions until I
request a cancellation.
I understand that the terms of this authorization will remain in effect until I request that they be
terminated. I acknowledge that I am responsible for making this request in writing to memberships@tennismasterusa.com
The Client understands that the “Credit Card Authorization Form” is required to be enrolled
entirely with Tennis Master Academies. Failure to sign the Agreement will result in a $100
penalty.
THERE IS NO CREDIT FOR TENNIS LESSONS. REFUNDS ARE GIVEN IN THE FORM MAKE UP
LESSONS. It is the client responsibility to make sure these lessons are scheduled for any
available day of the week. website.
Payment will be made to Tennis Master Academies LLC with an approved payment card or ACH.
Automatic payments are made the 1st of every month.
Each party has the authority to enter into this Contract and to perform all of its obligations under
this Contract information.
I fully agree to and understand the following
1. The student will be on time for scheduled tennis lesson hours.
2. The student will be prompt in attendance and not engage in skipping during any part of the
lesson.
3. Make-up lessons are given for sickness, vacations, and emergencies, as long as the “Credit
Card Authorization” is signed.
5. Make-up lessons have no expiration date for
7. Make-up days are offered on coaches and schedule availability. It is the parent/guardian’s full
responsibility to attend to the make-ups provided by the coach. Failure to do so will result in no
make-up for the missed lesson(s).
8. Clients must be enrolled in the current month to be eligible for make-up sessions for any
missed lessons from previous months. Make-up sessions cannot be scheduled or attended if the
client is not actively enrolled during the month in which the make-up session is requested.
PRIVATE LESSONS
1. Monthly fee must be active to attend make-ups (insurance required)
2. Max 4 make-ups can carry over to the next month
3.. No make-up for a missed make-up unless due to rain or weather
4. Make-ups are transferable within the same household – if the main student can’t attend, a sibling or parent can take the class instead
Coach Assignment for Private Lessons
*By registering for private lessons, the client acknowledges that they are not necessarily
assigned to a particular coach. TENNIS MASTER LLC reserves the right to switch or change the
assigned coach for any particular lesson as necessary.
*24-hour cancellation notice.
Student agrees to waive all rights to bring any and all legal action against Tennis Master
Academies employees and any employee, owner, agent, successor, managers, or assignee of
Tennis Master Academies for a claim of negligence or gross negligence regardless if the
negligence or gross negligence is attributed to Student, Tennis Master Academies or any
employees, owners, agents, successors, managers, or assignees of Tennis Master Academies.
Student agrees to waive any legal action against Tennis Master Academies, or any Tennis Master
Academies employees, owner, agent, successor, manager, or assignee due to any injuries,
monetary losses, deaths, or legal claims that may result from negligence or gross negligence,
regardless if such claims are attributed to Student, Tennis Master Academies , or any and all
employees, owners, agents, successors, managers, or assignees of Tennis Master Academies
Tennis Master Academies, owners, agents, successors, managers, or assignees are not liable or
responsible for any damages to any of the Student’s personal property in any form, regardless if
those damages to property are attributed to the negligence or gross negligence of Student,
Tennis Master Academies, or any employees, owners, agents, successors, managers, or
assignees of Tennis Master Academies , and Student waives any right to commence legal
proceedings as a result of such property damages.
Any legal disputes arising against Student or Tennis Master Academies, except for that of non-
payment, shall first be submitted to non-binding mediation as a condition precedent to any
litigation; the Parties stipulate to personal jurisdiction in the State of Florida and venue in the
County of Miami-Dade, or in the Federal District Court in and for the Southern District of Florida.
In the event of non-payment by the Student, Tennis Master Academies shall have the ability to
bring immediate legal action against the Student for recovery of all Studentship dues owed to
Tennis Master Academies.
Student agrees by signing this form that they have read this waiver and release in its entirety.
Student understands that they are assuming all the risk inherent in our Tennis Programs and
Clinics. Student understands that this is a release of all legal claims for damages, including
damages to person and personal property, that Student may have against Tennis Master
Academies for any negligence, gross negligence, action or inaction that is attributed to the
Student, Tennis Master Academies, or any employee, owner, agent, successor, manager, or
assignee of Tennis Master Academies. Student understands that this is the entire agreement
between Student and Tennis Master Academies and that it cannot be modified or changed in
any way by the representations or statements by any released party or by Student. Student
voluntarily signs their name as evidence of their acceptance of all the provisions in this waiver
and release and Student agrees to be bound by them.
THE UNDERSIGNED STUDENT, PARENT AND/OR LEGAL GUARDIAN ACKNOWLEDGES THAT
THEY HAVE READ AND UNDERSTOOD ALL THE TERMS AND CONDITIONS OF SERVICES OF
THIS AGREEMENT, INCLUDING THE DISCLAIMER AND RELEASE OF LIABILITY, AND THEY
UNDERSTAND AND VOLUNTARILY ELECT TO PARTICIPATE IN THIS ACTIVITY WITH
KNOWLEDGE OF THE DANGER INVOLVED, AND HEREBY AGREES TO ACCEPT AND ASSUME
ANY AND ALL RISKS OF PROPERTY DAMAGE, PERSONAL INJURY, OR DEATH.
All parties signing this Agreement acknowledge that they have the capacity to enter into this
Agreement and agree to be bound by the terms of this Agreement. The Disclaimer and Release
of Liability attached herein is made a part of this Agreement. This Agreement shall be governed
by, and construed in accordance with, the laws of the State of Florida and the federal laws of the
United States of America. Student agrees any disputes that arise regarding this agreement shall
first be submitted to non-binding mediation as a condition precedent to any litigation Student
irrevocably and unconditionally consents to submit to the exclusive jurisdiction of the courts of
the State of Florida and of the United States of America located in Miami-Dade County, Florida,
for any actions, suits or proceedings arising out of or relating to this Agreement, and Student
agrees not to commence any action, suit or proceeding relating to this Agreement except in
such courts. The language of this Agreement shall be deemed to be the result of negotiation
among the parties and their respective counsel and shall not be construed strictly for or against
any party.
The players and their parents’ consent to all videotaping and photographing of the player while
participating in Tennis Master Academies and/or on Tennis Master Academies employees
operated facilities. I agree that Tennis Master Academies can use images without prior approval.
The Business promises that it holds all licenses necessary to perform the work, that such
licenses are valid and effective as of the date any work is performed or services provided, and
that all work performed or services provided will be done in compliance with all applicable
federal, state, or local laws and regulations.
Each student or client is responsible for their belongings. Company is not responsible for any
items taken or stolen. If classes are cancelled by the Director by any reason, it would be
recuperated on Saturday of the same week, depending on the program you chose. If the client is
late more than 15 min for his lesson the professor has the right to reschedule another client or
give the lesson as given. Private lessons and clinics must confirm their attendance a day before
the activities. If a client signed up for the Clinics or Private Lessons and missed a lesson, it is the
responsibility of the client to notify and reschedule another day.
THE UNDERSIGNED STUDENT, PARENT AND/OR LEGAL GUARDIAN ACKNOWLEDGES THAT
THEY HAVE READ AND UNDERSTOOD ALL THE TERMS AND CONDITIONS OF SERVICES OF
THIS AGREEMENT, INCLUDING THE DISCLAIMER AND RELEASE OF LIABILITY ATTACHED
HERETO.
_Always wear the proper tennis clothing. (Clothes, shoes, and tennis bag)
– Students with long hair should put it up or wear a hat.
-After training, 15 min should be dedicated to stretch and cool down after each practice.
-It is highly recommended to reunite with friends on the weekends to play and keep improving
among the students of the academy.
– Silence is a very important training condition that must be respected and enforced in every
practice
– Treat the tennis equipment (balls, racquets, cones, fences, etc) with care like if it was your own.
-It is forbidden to have unsportsmanlike conduct before, during, and after practice. Throwing
racquets, swearing, or not following instructions will not be tolerated and the student will be
expelled from the training.
-any disrespect towards the coach or any other student will result in the expulsion of the student
who is not following the rules.
– There will be a “water break zone” where the student will be allowed to leave their belongings
(water bottles, bags, etc)
Parent’s Rules
-Parents are allowed to be present on training days, however, it is prohibited to be standing in
the court. They can be seated in the designated seat or where the coach considers they could
be seated.
-Parents should abstain to talk, motivate, pick up balls, celebrating, or showing support while the
students are training.
– Parents should abstain to make comments or interfere in the coaches’ training. If done so, this
will result in the expulsion of both parents and students from the class. Any comments can be
made before or after class.
– Parents may talk to the coach BEFORE or AFTER training, never during the session. This is to
avoid interruption of the training.
– Rainy days will be made upon the days the coach communicates it on our WhatsApp group.
Privates that are not able to be made up because of bad weather, will be exchanged with two
group sessions.
No shows for family events or vacations are not a motive for make-ups, but the academy will do
provide make-up options as long as we have spaces in other groups.
– On breaks (Winter Break, Spring Break, etc) it is suggested that students come to the
programs offered to have an “intensive of tennis”, designed for high-performance students that
will enhance their physical and technical skills.
Clients must notify TENNIS MASTER LLC of any cancellations or requests to freeze their
accounts at least 15 days in advance. Notification must be provided to both the coaches and via
email to memberships@tennismasterusa.com. Failure to provide the required notice will result in
the continuation of membership fees.
In the event a client cancels or closes their account and subsequently wishes to reinstate their
membership, a reinstatement fee of $50 will be applied.
By signing this agreement, the client acknowledges that they have read, understood, and agreed
to all the clauses, rules, regulations, and payment procedures of TENNIS MASTER LLC. The
client affirms that they are fully aware of their responsibilities and obligations under this
agreement.
Since every month is different is regards of the number of weeks, (some months have 5 weeks),
we only will be charging a FLAT FEE (depending on which programs was chosen), regardless of
how many times the athlete comes to practice in any particular month.
There may be exceptions to this contract due to verified extended illness or other special
circumstances. In such cases it is important that the parent(s) communicate with the coach and
follow established academy guidelines for extended absences.
The purpose of these policies is to make all students accountable for good attendance and to
achieve a quality learning and fun environment.

Academies
